What's PLDD laser surgery?

Percutaneous laser disc decompression (PLDD) is a type of surgery in which a laser probe is inserted into the intervertebral disc space and laser energy applied for achieving decompression and neuromodulation thereby relieving the pain. PLDD is a minimally invasive procedure that falls into the category of percutaneous intervertebral surgeries with the aim to significantly reduce the patient's pain and recover neurological deficit. It is performed under local anesthesia. By a specially designed laser, with a coefficient of absorbing energy adjusted for soft discus tissue, specific amount of heat is sent to achieve the evaporation of the water from the disc without additional thermal damage thus achieving decompression and creation of a stable intradiscal scar that will prevent herniation from reoccurring.


PLDD is a minimally invasive laser treatment performed on an outpatient basis using only local anesthesia. During the procedure, a laser beam vaporizes a tiny portion of the disc, decompressing it. Patients typically get off the table and go home. There is no hospital stay, no long recovery period, and PLDD costs many thousands of dollars less than surgery.


The PLDD treatment is performed on an outpatient basis using only local anesthesia. During the procedure, a thin needle is inserted into the herniated disc under x-ray guidance. An optical fiber is inserted through the needle and laser energy is sent through the fiber, vaporizing a tiny portion of the disc nucleus. This creates a partial vacuum which draws the herniation away from the nerve root, thereby relieving the pain. The effect usually is immediate.

Patients get off the table with just a small adhesive bandage and return home for 24 hours of bed rest. Then patients begin progressive ambulation, walking up to a mile. Most return to work in four to five days.

Because only a thin needle is used, there is no cutting and no scarring. Since only a tiny amount of disc is vaporized, there is no subsequent spinal instability. PLDD is different from open lumbar disc surgery because there is no damage to the back muscle, no bone removal or large skin incision. Most of the complications that may occur with open surgery are eliminated with the PLDD procedure.

The theoretical objective is to reduce the intradiscal pressure in the nucleus pulposus in such a way that the herniated fragment can redirect to the interior of the disc, reducing compression of the nerve root, which reduces pain, and therefore improves the patient's morbidity and quality of life.

Therefore, this approach consists in eliminating the nucleus pulposus by applying laser energy in the center of the intervertebral disc that produces a vaporization of its aqueous content. Being a minimally invasive technique, it is expected that the damage in the tissues (muscles, bones, ligaments and nerves) is the least possible.
It is a procedure that can be an option for different states of disc degeneration and although it can be applied in all regions of the spine, it is applied with higher frequency in lumbar disc hernias. This is a safe procedure

PLDD procedure is performed using local anesthesia. Optical fiber is inserted in special cannula under fluoroscopic guidance.After

applying contrast to the facet it is possible to check cannula's position and the condition of the disc bulge. Starting laser
initiates decompression and lowers intradiscal pressure. The procedure is done from the posterior-lateral approach with no
interference to vertebral canal,therefore,there is no possibility of damaging a reparative treatment,but there is no possibility
to reinforce annulus fibrosus. DUring PLDD disc volume is minimally decreased,however,disc pressure can be significantly lowered.
In case of using laser to disc decomperssion,small amount of nucleus pulposus evaporates.

The procedure appear to be now days a safe and valid alternative to the microsurgery, with a rate of success of 80%, particularly under CT-Scan guidance, in order to visualize the nerve root and also apply energy on several points of disc herniation. This permits to have a shrinking concentrated in a bigger area, realizing a minimal invasiveness on the spine to be treated, and avoiding the potential complications related to the microdiscectomy (recurrence rate of more than 8-15%, peridural scar in more than 6-10%, dural sac tear, bleeding, iatrogenic microinstability), and does not preclude traditional surgery, if needed.
