The most important feature of our laser diode is its dual wavelength which allows it to be used for a variety of tissue interactions. 980nm(max 30w laser power) and 1470nm (Max 17 w laser power ).
980nm are absorbed in the first instance by hemoglobine (HbO2) whereas longer wavelength like 1470nm are dominantly absorbed by intracellular water.

For minimally invasive endovenous laser ablation - EVLT,TRIANGEL diode lasers emitting radiation at different wavelengths: 980nm,1470nm with the possibility of their further expansion with additional light sources.
As can be seen in the diagram, for 980nm radiation, the absorption coecient is higher for hemoglobin than for water. For 1470nm, the opposite is true and water absorbs light radiation many times better than hemoglobin. Both of these chromatophores are components of human tissues, including blood and vascular walls. Lasers with a wavelength of 980nm have been used on a large scale for over 20 years and the average power used during treatments is 10-12W. Using 1470nm wavelength, smaller powers are needed - at 6-8Wto perform an effective surgery. Endovenous occlusion of great saphenous vain and small saphenus vain

The procedure is performed in the clinic with topical anesthetic cream and takes approximately 30-45 minutes. There is minimal or no pain and no downtime, the patient can resume normal activity immediately. This treatment should be performed 6-8 weeks apart and for best results, avoidance of direct sunlight to the treated area is recommended.

At 1470 nm, the absorption coefficient of water (in the vascular wall) is considerably greater than it is between 810 nm and 1064 nm. Laser energy can thus be controlled much better than with shorter wavelengths. Lasers are therefore primarily used for the benefit of patients. Since 2013, TRIANGELMED has exclusively been using a wavelength of 1470 nm for EVLT treatment.



360° Radial Fiber
 
Radial fiber that emits at 360° affords the ideal endovenous thermal ablation. It is therefore possible to gently and evenly introduce the laser energy into the lumen of the vein and ensure the closure of the vein based on photothermal destruction (at temperatures between 100 and 120°C).
TRIANGEL RADIAL FIBER is equipped with safety markings for optimal control of the pullback process.

When combined with the 1470 mm laser source, this laser fiber that emits at 360° affords the ideal endovenous thermal ablation. 

 Perforation of the vein wall (as in the case of the conventional bare tip fibre) and the associated thermal irritation of the surrounding tissue are prevented, thereby minimizing postoperative pain, ecchymosis, and other side effects. The 360 Radial fiber (atraumatic fiber tip) is guided directly into the vein via a short introducer. Thanks to excellent visibility in ultrasound, the fiber tip can be optimally monitored and placed.

In TRIANGEL EVLT treatment an optical fiber is inserted into the to-be-treated vein, and the laser energy is directed into the interior of the vein. The vein contracts as the vein wall is destroyed while the optical fiber is slowly withdrawn. This leads to occlusion of the vein and the healthy veins that surround the closed vein can then restore the normal flow of blood to the treated area.


The procedure is preformed under local anesthetic and can be completed in just under an hour, depending on the size and number of varicose veins. The general steps of the treatment are as follows:


Step 1 - ACCESS: Puncturing of the vein with the needle under ultra sound control creates an entry hole in order to allow the laser fiber to be inserted into the vein through a catheter. Ultrasound is also used in order to properly position the fiber in the vein.

Step 2 - PROTECT: Tumescent anesthesia is delivered through a syringe or by using a special pump. Tumescent anesthesia has analgesic and vasoconstrictor effects, also it creates a cold shield in the vein to protect the surrounding tissue.

Step 3 - OCCLUDE: Laser energy from 980+1470nm laser is delivered to the desired location inside the vein by using a bare laser fiber inside a catheter. The laser is repeatedly fired as it is gradually withdrawn, depositing the thermal energy into the blood and vein wall.

EVLT (Endovenous Laser Treatment) is a procedure leading to occlusion of varicose veins. It involves putting a fiber optic into a saphenous vein through a catheter. Then the laser is turned on and slowly withdrawed from the vein. Thanks to light-tissue interaction mainly thermal effects occur, the tissue is heated and the walls of the vein shrink, because of alteration of endothelium and contraction of collagen. 

There are two possibilities of performing the treatment: with pulsed and continuous-wave laser operation. Using the pulsed operation also the fiber is withdrawed step by step. A better choice is to use continuous-wave laser and to withdraw the fiber also continuously, what provides more homogenous illumination of the vein, less tissue damaged outside the vein and better results. The therapy is just a beginning of the occlusion process. After the treatment the veins are shrinking for several days or weeks. 

That is why in the long period observation very good results are obtained. The main advantage of EVLT is that it is a non-invasive technique in contrast to surgical treatments. It also does not involve hospital stay and can be performed in an ambulatory conditions with local anesthesia andlasts for less than 1 hour. After the procedure patient does not haveany unsightly incisions and scars.

Thanks to radial illumination the maximum energy density is directed to the walls of the vein near the fiber and cause immediate occlusion. The simple bare fiber is also possible to use, but the treatment is much more effective with the radial illumination.
